Apple has released information on how to apply for your $100 store credit if you were an early iPhone owner.

To receive your credit, you must complete a form at Apple's website. You will then receive an SMS message with an access code needed to generate an electronic store credit. The store credit can be used at either the Apple Store online or any Apple retail store.
