My 3G started doing this exact same thing over the summer. I had Apple replace it as I was under the 1-year warranty still. It's definitely a hardware problem, not a software problem. Take it to them, they'll be able to fix it/replace it but it will definitely cost you.

Apple's Support Site has some applicable FAQs: http://www.apple.com/support/iphone/service/faq/

Unfortunately, if it's out of warranty, you can't get extended warranty now:
Can I extend service coverage on my iPhone beyond the warranty?

The AppleCare Protection Plan for iPhone will extend the hardware repair coverage to up to two years from the original purchase date of the iPhone. The AppleCare Protection Plan for iPhone must be purchased while the iPhone is covered under Apple's Limited Warranty for iPhone.

And, Apple's website also tells you what your options are since it's out of warranty. It looks like it'll cost some money. Alternatively, maybe you can find a good repair shop?

You can always try putting a drop of alcohol in the vibrate switch to clean out anything that may have gotten in it.
