App for custom affirmation?

I have searched a lot but only found apps with built in affirmations. Can you please recommend any app which allow me to enter/edit my own affirmation?

I think I haven't clarified my need enough. Here is what I mean by affirmation:

I usually read lots of book and I like to remind myself regarding lots of my bad habits so that I can improve them. One excellent way for this is to write down the positive thought you have and review them daily.

For example, here are something I want to get reminded:
* Drink 3 ltrs of water daily
* Walk with confidence
etc.. etc..

I want to write down all these in my iphone and the app should show this to me randomly whenever I tap on the screen.

Let's say, in the future I have started doing what I have written then I should be able to mark it as done and the app shouldn't show that affirmation again.

The app which closely match to my need is MyThoughts (http://www.knowledgebrain.com/mythoughts.html) but unfortunately it is poorly designed. So, I'm looking for something much better.
