OK, I purchased the iphone 3G July 12, 2008, so I am assuming I was an "early adopter". So do I qualify for the $199 16 gb 3G S?

Yes, I did that and it gives me the 399$ price, but I keep reading stuff that says those who bought a 3G July-Sept of last year get the 199 deal, so I'm confused.:gasp:

Instead of using the website, call 1 (800) 331-0500 and explain your situation. You'll find out for sure.

Then you will be able to. Not ALL of the call reps are really up-to-date with what's going on.
But if your plan is atleast $99 a month (you said yours was) then yes. They might have not updated this to include a search of plans in the process but if you're not far from a store a) ask if they have any left b) just go there if they do... do it in person. MUCH faster and you'll get your phone then.
