If you are on a mac it can be done with ical. Not sure about windows applications.

I have an appointment which is scheduled every 4 weeks; monthly doesn't work for that either.

When you go into repeat in the event do you click custom? It has all sorts of ways to set up an event.

Custom? Are you referring to iCal? On iPhone the only options for repeat event are:
Non, Every Day, Every Week, Every 2 Weeks, Every Month, Every Year.

After reading the OP title carefully, I see it was meant from the phone,in which case it is limited. If that is the only way you have to set up an event it makes it tough. But you could set it up in ical if one had it and sync to the phone.

plus i'm also on windows so no ical for me... :(
unless a windows ical compliant program like mozilla's sunbird can then be imported?

I was excited when nemussync hit the app store because it allows you to sync google calendars with your iPhone over the air.
Unfortunately google calendars doesn't even have this level of sophistication so we windows users are still stuck out.
editorial:
This is precisely the kind of thing that makes me dislike Apple. They remove necessary functionality for the sake of simplicity. Function follows form.

Do you have Outlook?
If you use outlook you can set up weekly, monthly, and yearly reminders and even use different reminder times for events. When you set them up in outlook and sync they will be recognized on the iphone.
