Contacts app on home page

I am looking for the source for the app that puts an icon for Contacts on the home page as well as the one that replaces iPod with a video icon and a music icon. Thanks

the music and video mod can be found at

look for the "ipod2" mod. this will create a new music and video icon, and still keep the ipod icon.

if you want to create a new music and vid icon and remove the ipod one, i think you need the "ipod1" mod.
